TERMS AND CONDITIONS OF VOLUNTEERING: By completing this volunteer registration you understand and agree to the following:
VOLUNTEER RELATIONSHIP: You understand that you are agreeing to provide volunteer services and engage in activities as a volunteer. You understand that your relationship to Animal Voice Advocacy (“AVA”) will be that of a volunteer, no compensation is expected in return for your services, and you will not be an employee of AVA.
ASSUMPTION OF RISK: You represent that, to your knowledge, you are in good health and have no physical or mental impairment that would or should prevent your participation as a volunteer with AVA. Volunteering with AVA involves some risk of injury. Injuries include, but are not limited to, those caused by slips and falls, improper lifting of heavy objects, interacting with animals (both domesticated and wild) as well as potentially the improper use of tools or machinery. You understand and accept this risk and agree that AVA will not be liable for any injury to you or property damage sustained while volunteering.
WAIVER AND RELEASE: You agree to release and forever discharge and hold harmless AVA from any and all liability, claims and demands of whatever nature, in law and in equity, which arise or may hereafter arise from your activities as a volunteer with AVA wherever, whenever, or however the same may occur. You understand that this Release discharges AVA from any liability or claim with respect to bodily injury, death, property damage resulting from AVA services, fundraising events, public awareness activity, or that occurs will you are providing volunteer services to AVA.
INSURANCE: You affirm that you are covered by primary medical insurance and understand that you are responsible for your own medical bills if injury occurs. You understand that AVA does not carry insurance of any kind for your benefit and does not assume any obligation to provide you with financial or other assistance included but not limited to medical, health, or disability benefits or insurance of any nature in the event of injury, death or damage to property.
MEDICAL TREATMENT: You release and forever discharge AVA from any claim which may arise on account of any first aid treatment or other medical services rendered in connection with an emergency during your time as a volunteer with AVA and you hereby consent to AVA providing, administering or obtaining medical treatment for you in the event of an injury while acting as a volunteer with AVA.
DRUGS AND ALCOHOL: To maintain a positive environment for AVA’s programs and participants, volunteers are prohibited from using, distributing, being in the possession of, or under the influence of illegal drugs or alcohol while serving in a volunteer capacity. You agree to abide by this policy.
I understand that this document is a contract that eliminates any liability of AVA for my activities as a volunteer and that I agree to indemnify and hold harmless AVA for all claims arising out of my participation as a volunteer with AVA. I agree to execute this Release willingly and voluntarily.
